and return the Fundraiser Initiation & Agreement
Form and mail or FAX to our office.

Each item
in the catalog is $10.00. Your Group will keep
50% of all money raised through sales.
For example, if you sell 10 items for a total
of $100, your Group keeps $50 instantly!

Your Fundraising
Sale will last for two full weeks. At the end
of the sales period, calculate your share (50%
of the amount raised!), and mail us all of the
original order forms along with one check for
the remainder of the amount raised payable to
Quantum Mfg. Payment must be submitted with the
orders. (Remember, you keep 50% immediately.)

We
will cover shipping costs back to your organization
if you sell over 50 items (over $250 paid to Quantum).
On fundraisers resulting in a payment
of $250 or less to Quantum, the shipping cost
will be a flt rate of $10. Please include this
amount in your payment to Quantum at the end of
your fundraiser.

seller's order is individually packaged
with the order form- NO CONFUSION! An organizer's
dream come true!

Please allow
3-4 weeks for delivery. Missing orders and damaged
items must be reported to us within 7 days of
receipt
